Admission Requirements

  • Must be 5 years old on or before September 30 to enter Kindergarten
  • Birth Certificate
  • Health Records
    • State requirements mandate that a “universal” form with an expiration date must be acquired from a physician or parish health unit and submitted before the first day of classes
      • REQUIRED FOR CHILDREN OVER AGE 4:
        • Varicella (chicken pox) – 2 doses
        • Measles, mumps, rebella (MMR) – 2 doses
        • Hepatitis b (HBV) – 3 doses
        • Tetanus, diphtheria, acellular pertussis (whooping cough) vaccine booster
        • Polio virus vaccine booster
      • REQUIRED FOR CHILDREN ENROLLED IN 6TH GRADE:
        • Tetanus, diphtheria, acellular pertussis (whooping cough) vaccine booster
        • Varicella (chicken pox) vaccine – 2 doses
        • Measles, mumps, rubella (MMR) vaccine – 2 doses
        • Meningococcal vaccine (bacterial meningitis) – one dose
        • Hepatitis b (HBV) – 3 doses
        • Polio virus – 4 doses

MEETING OUR STUDENTS’ NEEDS

We cannot serve every student who applies for admission.  Our small classes, caring environment and dedicated teachers allow us to help most students learn and grow, but some students are in need of specialized assistance.  Trinity Lutheran School does consider all requests for reasonable accommodations, but it is limited in its ability to serve students.  The responsibility to inform TLS of the nature of a student’s disability and request an accommodation lies with the parent or guardian.  On a case-by-case basis, the school principal will consider the school’s ability to meet the needs of prospective students and discuss options for accommodations during the family interview.
